No. The Dementia Care Navigation Program is a non-medical care advisory and coordination service. We do not provide clinical evaluation, diagnosis, or medical treatment. We help families understand their options, organize their approach, and connect with the right resources — including medical providers when appropriate.
As early as possible. The period right after a diagnosis is when families have the most options and the most time to plan well. Early navigation helps you avoid reactive decisions made in crisis and gives your loved one a more stable, supported experience at home.

Yes. When applicable, we help families connect to the GUIDE Dementia Care model — a CMS-supported program that provides structured dementia care resources for eligible individuals. We will discuss whether this applies to your situation during your consultation call.
Many families continue with monthly advisory support as their loved one's needs evolve. Others transition into ongoing in-home care through HomeWell Care Services. We will discuss the right next step for your family as the program comes to a close.
No. The Dementia Care Navigation Program is a completely standalone advisory service. You do not need to commit to in-home care to participate. Many families use the program to decide whether and when in-home care is the right next step.
